A 64-year-old Caucasian female presents to the clinic to see the Family Nurse Practitioner with a three-hour history of shortness of breath and substernal chest pain. She was discharged five days ago from the hospital after having a total hip replaced for severe osteoarthritis. The hip surgery was uneventful. On physical examination, the patient appears anxious with a respiratory rate of 36 breaths/minute. Her temperature is 100.0 °F, blood pressure is 100/70 mm Hg. Auscultation of the lungs includes adventitious breath sounds in all lobes and decreased breath sounds at the right lower lung base. The patient is also concerned about returning to the hospital since she is her husband’s primary caretaker, who has dementia. During her hip surgery, a friend from her church stayed with her husband, but she does not want to ask for additional help and is refusing to go to the emergency room.
