Patient Encounter: You are lead physician assistant at the Blue Marble Medical Clinic, with authority to act as a physician. A 54 yr old white man comes to the office because of a 3-day history of worsening cough that is productive of a moderate amount of yellowish, non-bloody sputum. He says the cough causes sharp pain in the right side of chest, and the cough and pain have been causing difficulty sleeping. He has not had anything like this before. Patient felt fine until about 4 days ago. He thinks he has had a fever but did not take his temperature. He had a shaking chill last night that shook the bed and alarmed his wife. He feels quite “run down” and was too sick to go to work today so he came in. Prior to onset of this illness he felt fine and was doing all his normal activities. He has been eating normally with no recent weight loss. Denies noticing rash or skin lesions.
