Reword. Phone call from PCG updating SW on Choices and tenn care. PCg reported choices representative called and completed assessment via phone due to inclement weather and she had to sign forms and other documents to send back. PCg reported she recieved approvals from tenn care and choices. SW educated pcg on once her coordinator is assigned utilizing resources fro the full number of approved hours and ensuring she explained fully the need of pt. SW educaed with the example that some time patient is able to feed himself then other days as a result of his disability he is not sure of how to feed himself. sw also inquired if pcg has heard anything from Social Security Administration concerning representative payee information. PCg reported nothing yet but she will go to the office when the weather breaks. sw also will provide pcg a copy of representative payee paperwork fr her records. pcg expressed gratitude for caris team
