Concorde Hospital Physician Orders: Evaluation and treat, TDWB left LE Patient Name: Margie Smart Primary Diagnosis: Fx left hip and Fx left distal radius HPI: Patient sustained a fracture to left hip and left wrist after fall when dog jerked the leash chasing squirrel. Patient underwent ORIF of left hip and closed reduction of the left wrist Social Hx: Patient is a 72 year old Caucasian female, retired teacher that lives alone in a two story home in mid-town. The patient’s bedroom is on the second floor and the laundry is in the basement. Prior to fall, patient was independent in all ADLS and volunteered at local pet rescue. The patient has smoked a pack a day for the last 30 yrs with moderate alcohol use. Patient has two sons, one of which lives nearby PMH: Secondary Dx: OA, stenosis, COPD, pedal edema, peptic ulcer disease, stress incontinence Medications: Coumadin (1 tablet 1x/day) Lovenox 40mg SQ every day, Demerol
