Skin assessments are an important part of the nursing profession. When individuals enter a medical facility for an extended period, activity decreases significantly. Decreased activity with weight on bony prominences can lead to skin breakdown, something that can be easily prevented by assessing the skin at least every shift. According to Bass (2023) The Agency of Healthcare Research and Quality estimates 2.5 million Americans develop pressure injuries annually. Pressure injuries have the potential to decrease quality of life and increase hospital stays, it is a nurse’s duty to give high quality of care while an individual is in the inpatient setting. Quality improvement projects have taken place to determine the validity of the two nurse skin assessments (Snider, 2021). Increased communication to determine the severity of skin breakdown increases accuracy and allows medical staff to provide adequate care for an individual’s needs. paraphrase
