He was diagnosed with congestive heart failure a few years prior. He visits the emergency department frequently to receive care. He does not qualify for Medicaid and struggles to pay all his expenses with his current pay, so he cannot afford medications to treat his condition. A standard used by the Institute of Healthcare Improvement is the Triple Aim (2013). The three aims are improving patients’ experience of care, improving population health, and reducing the per capita of healthcare.
