RH is a 44-year-old man with a history of hypertension who was recently diagnosed with ischemic heart disease after complaining of increasing chest discomfort while exercising. He presents to the clinic to follow up after his hospitalization for chest pain with noted troponin elevation. EKG unremarkable, diagnosed with NSTEMI, cardiac catheterization confirmed 85% occlusion in the left anterior descending artery (LAD). Single-vessel disease with no other occlusion noted significantly. A stent was placed. Further noted confirmed new history of hyperlipidemia with significantly elevated LDL, total cholesterol. He denies having chest pain since the first incident of discomfort that precipitated his hospitalization. He indicates no previous history of myocardial infarction, stroke, or peripheral artery disease. He has no siblings, both parents are alive, and his mother had an MI when she was 47 years old and has elevated cholesterol. He is married with two children. He does not smoke and exercises regularly
