A patient is visiting the United States where her 19 year old son lives. All of the patient’s extended family lives in another country. The patient has a massive stroke and has sustained catastrophic brain damage. The patient is not responsive and is unable to breathe without the assistance of a ventilator. The son believes that his mother would not want to live like this. The son is being asked to make the difficult decision to remove his mother from life support as there is no expectation for recovery for her. The extended family has been in constant contact with the son and are pressuring him to demand a trach and PEG to keep her alive and not “not give up on her”. The extended family believes that medical care in the United States is so advanced and so much better than in their home country that surely there is a way to help her recover. They wonder if the providers are not providing optimal care due to the fact that the mother is not a U.S. citizen and does not have the financial means to pay the hospital bill.
