• Mr. Solomon is a 76-year-old male with chronic obstructive pulmonary disease (COPD) and Alzheimer dementia who was admitted with increasing shortness of breath. He has been living at home with his wife but recently has been having increasing episodes of confusion and disorientation to self, place, and time. He is unable to communicate his level of pain or discomfort. His vital signs at 0400 were T 37.2° C (99° F), P 102 and regular, R 24 and labored, BP 134/86, with a pulse oximetry reading of 88% on 2 L of oxygen by nasal cannula. • Mrs. Jarvis is a 45-year-old female on postoperative day 1 after an abdominal hysterectomy. She has a Foley catheter and an intravenous (IV) line. Overnight, she received morphine twice for pain rated as 8 of 10. She has the original abdominal dressing in place. She was afebrile overnight, and other vital signs were within normal limits. • Mrs. Kimball is a 92-year-old female with urosepsis who was admitted from home 3 days earlier.
