A 52-year-old man with Down syndrome presents to his primary care provider with questions about long-term planning for his future. His past medical history includes repair of a congenital cardiac defect shortly after birth, but otherwise his health history is unremarkable. The patient has a brother who has been helpful in his care over the years but lives out of state; the patient has no other living family and has been independent for most of his adult life. On review of his history, the patient is able to articulate his remote childhood health history but is vague in describing any current complaints. The clinician becomes concerned about his vague responses and begins a mental status examination. Among other findings, he believes the year to be 5 years after his birth and while he is able to state his full name, he is unable to tell what location he is presently at, naming the city of his birth in another state. The clinician is concerned about his safety and future given these findings. Which of the following is the best next course of action regarding this patient? Call the brother out of state even if the patient declines intervention from family
