Major Prevention Case Study Mr. White Identifying Data: age 50 year old male HPI: A new patient to this clinic. He has been without health care for 4 years because no insurance. Here today to establish care now that he has insurance. Current Health Data: Medications- none Allergies- none Screening tests/last PE- about 5 years ago—told all was normal Immunizations- flu shot—1 year ago at work. Others are unknown. Occupation: Truck driver PMH: Childhood illnesses: none reported Adult illnesses: none known; was told he was overweight Hospitalizations/ Surgeries: none Sexual Hx: sexually active with men only; no long term partners. Inconsistent condom use. Has approx. 5-6 partners annually. Has had some STIs in recent past but cannot name them. Psych Hx: sad & lonely sometimes, no psych diagnoses Family Hx: Father (62 y.o.) with HTN, prostate Cancer, Type 2 diabetes; mother (60 y.o.) with recent diagnosis of breast cancer Personal/Social Hx: cigarette smoker approximately 1 PPD, 3 beers every evening, lives alone in an apartment. Driving now for a major trucking company and has good insurance. PE: VS- P-62, R- 18, BP-138/72, ht- 5’4, wt 190, BMI- 32.6
