A.L has been admitted to the unit with signs and symptoms of dehydration and malnutrition. The patient verbalized not eating a whole lot since his wife has passed. A.L also states he mostly eats soft foods due to not having his dentures and since his stroke he has right sided weakness. The patient has dry oral mucosa, dry and tenting skin which are signs of dehydration. Low blood pressure and elevated heart rate can be caused by dehydration as well. Lab values show dehydration and malnutrition. High Hct, Na, Cl, and Cr show that the patient is dehydrated and has low urine output which also is a sign of dehydration. Low Hgb, RBC, Prealbumin, Albumin and Transferrin are signs that the patient is malnourished. Hypernatremia is excessive sodium in the body. With this condition the early signs and symptoms are nausea, vomiting and diarrhea and later symptoms are confusion and delirium. The patient has bruising from his fall but has also been taking anti-coagulants which increase the chance of bleeding. The patient also has a pressure wound to his right scapula and right buttock which implies that he has been laying on that side for some time. Nutrition plays in
