Can you reply to this classmate? With that being said, I feel that the earlier in the process the patient and their family are exposed to the options that are available, the easier the process becomes for them. There is always initial shock behind the discussion, but with time and explanation, the patient and family are able to make informed decisions regarding their care and the direction they want to take. Having been involved in EMS and emergency nursing, I feel that a direct approach is always best. I would speak to the patient and his or her spouse or closest relative alone first. A small controlled setting would allow for questions and education for the patient and their family without the charged atmosphere that sometimes comes when multiple family members are involved. Then as a team, we would discuss the patient’s illness and prognosis, and then what their goals are for treatment and management. Giving them a clear and concise view of how their illness will progress often helps with decision making, and most patients are grateful for honesty and clarity. Then, once they have made a decision, I would advocate for them to family and other providers that
