1) What labs would you order for this patient? • Due to a history of Hepatitis C and the possibility that drugs could affect the liver, liver function tests (LFTs) to check how well the liver is working. • Do a full blood count (CBC) • HbA1c to check on long-term control of blood sugar. • A lipid screen to measure cholesterol levels. • Hepatitis C viral load 2) What would you consider treating his pain with? • Because of the patient’s past and the chance of drug combinations, opioids like Vicodin should not be given to him, especially since he has used drugs through an IV in the past. Painkillers that aren’t opioids, like Acetaminophen at reduced dosing 2-3g/day are a safe choice. 3) How would you manage his Blood Pressure? • Based on the patient’s history of high blood pressure and the most recent results, the antihypertensive medication may need to be changed. Given his other health problems, a drug that treats both high blood pressure and those other problems (like an angiotensin-converting enzyme inhibitor or angiotensin II receptor blocker) might be a good choice. 4) What would you treat his Cholesterol with considering his Hepatic function? •
