45 years old caucasian male presented to ED with right ankle pain/ deformity after ground level fall at home. Patient reports about an hour prior to arrival, he slipped on a wet tile in his foyer at his home. He reported immediate right ankle pain and was not able to weight bear on his right lower extremity. He denies any head injury, loss of consciousness or anticoagulation therapy. Patient was evaluated upon arrival in the emergency department. X ray of the right ankle and right foot revealed a right ankle fracture, dislocation of the lateral and medial malleolus. Ortho was consulted for evaluation and management with closed reduction under procedural sedation of his right ankle. Physical Therapy Evaluation: Supine to/From sit, sit to / from stand. Ambulated 20 feet x 2 axillary crutches SBA/CGA ascend/ descend 2 steps with unilateral handrail and crutches CGA o Finding: Patient navigated in the room and outside hallwall with B axillary crutches at SBA/CGA with cues for slower velocity. Patient was able to hop 2 – 8 inch steps with unilateral handrail and crutches at CGA level o Impression: Patient tolerated well. Patient would benefit from further skilled therapy to maximize functional independence and safety. Discuss Gerontological Considerations Identified
