A 32-year-old woman is taking Lithium carbonate 600 mg capsule by mouth twice daily for bipolar I disorder. She has been stable on this dose and has not been hospitalized for the past three years. She recently found out she is 12 weeks pregnant. Her OBGYN said she could continue taking Lithium during pregnancy and discussed the risk of Epstein’s anomaly but would like her psychiatric provider to manage her Lithium during pregnancy. How would you manage her Lithium during pregnancy? How often would you monitor lithium levels? What changes to dose would you expect during pregnancy and in the immediate postpartum?
