In a one-on-one clinical interaction the nursing instructor questions an LPN-to-RN transition student with regard to the priority of care. The student is unsure why the patient is still on a monitored floor and is sent to investigate. On return, the student explains that just 2 days ago, the patient had pulmonary edema from fluid volume overload, and, with the change in medications the patient received, the physician required that the patient be closely monitored to ensure that the pulmonary edema did not recur. The instructor then asks what the patient’s intake and output have been for the previous 24 hours and what the patient’s weight has been. The student reports that the patient has not been saving the urine for the nurse to measure and that the weight check was not done this morning.
