Reword.SW explained to PCG that social worker is not afillated with Caris healthcare. SW then explained to PG that patient are often denied Tenn Care for various reasons and sometimes Sw has to appeal the decision several time for various reasons. PCg further went on to say that the patient was fine that night she has a visit from skilled nursing and the nurse recommended the medication. PCg stated she cant help but womder if the patient was over medicated which cause the decline so quickly. SW referred pcg to skilled nursing for additional information and to help clear up any questions or concerns. Sw also educated PCg on the various stages of grief and that this is a part of the grieving process to have so manhy question and wonder what she could have done differently. pcg expressed gratitude to SW for all her hardwork.
