A 28 year-old female with MDD presents to a follow up appointment with the psychiatric nurse practitioner after a dose increase one month earlier from 20mg to 40mg daily fluoxetine. She reports full medication adherence without side effects and an improvement in dysphoria following dose increase, saying “I’m about 50% better, but still having trouble with wanting to get out and do anything.” As the PMHNP, you: a. Understand that 50% improvement is equivalent to symptom response. b. Decrease 40mg to 30mg dose, as you suspect the patient is experiencing apathy secondary to supratherapeutic dosing c. Recommend continuing 40mg daily and suggest twice-weekly therapy as adjunct for refractory symptoms d. Continue 40mg daily, with the understanding that 50% improvement is equivalent to symptom remission.
