Past Medical History Patient states that he was diagnosed with atrial fibrillation eight years ago. Patient denies having other condition such as heart attack, heart failure. Patient states that his blood pressure is usually on the higher side and takes anti- hypertensive medicine to control it. Patient states his LDH levels are high and his PCP had prescribed him medicine to lower his cholesterol level. Patient denies having any history of diabetes mellitus. Patient denies having any respiratory disease. Patient reports that he is allergic to bee sting and was hospitalized due to the reaction. Patient describe having chest tightness and difficulty breathing and was rushed to the hospital where he was treated with epinephrine. Patient states that he does not recall the last chest x-ray done but reports that “it was normal”. Patient reports that he has received all his corona virus vaccines and booster dose. Patient states that he had his pneumococcal vaccine last month. what can i conclude from this history
