Mr. Price is a 42-year-old male who presents with c/o nausea and vomiting. This started the night before last a couple of hours after eating dinner. He states the vomit was “normal looking,” which he means contained digested foot and has not been associated with meals. He has not eaten much in the last 36 hours. He also c/o cramping pain that is near his umbilicus but is poorly localized. The pain is severe, 8/10, and he gets some relief from the pain after vomiting. He feels generally unwell and is unsure if he had a fever. He has no night sweats and no sick contacts. No recent travel. He moved his bowels yesterday, but he has no gas or stool today. PMH- None PSH- Appendectomy 20 years ago Non-smoker drinks 1 bottle of wine weekly, occasional cocaine use, last use 4 days ago. Tylenol occasionally for headaches, he uses some herbal remedies and aromatherapy. Physical Exam: T- 101, HR 104, BP 100/60 RR 16 HEENT- Neg. Neuro- Neg Respiratory- Neg Cardiac S1 and S2 regular, tachycardia GI- Appy scar, firm and distended, pain all over, not localized, high-pitched BS, no palpable masses. GU- voided earlier in the am.
