Sam, 14, has been living in the disability home for the past year after his mother, Tamara, couldn’t care for him full-time. Sam is diagnosed with 18q Chromosome deletion, Intellectual Disability, Autism Spectrum Disorder, and a Secondary Diagnosis of Seizure Disorder/Epilepsy. Sam experiences hay fever and has a dust allergy. Sam has sensitive hearing and becomes agitated with loud volumes and sudden noise. Historically, he has been reported as volatile and unpredictable with moods and behaviours that have resulted in physical assaults on his mother and carers. Sam likes to do arts and crafts in the front room of the home, which is quiet, spacious and has lots of natural light. Sam enjoys doing activities with his Personal Care Worker. Sam has just been disturbed by a car going past with a high-revving motor and loud exhaust, which backfired. He has started rocking in his seat, hitting himself in the head quite violently. He has drawn blood from near his eye and temple, which he is unaware of. He was about to do some arts and crafts and still has a small paintbrush in his hand1. When did the incident occur?
