Baby Girl L. is the 3800 gram product of a 37 weeks by dates gestation. Her mother is a 37-year-old woman who lives in a rural area with little access to routine health care. She has been pregnant 6 times, has had 3 miscarriages (spontaneous abortions) and three previous live births. One of her babies died in the first month of life. At that time, her family was living in a refugee camp and mother is not able to say what the cause of death was. During this pregnancy, mother has had three prenatal visits but no ultrasound. She reports that she has had no infections, has not used drugs or alcohol and has felt the baby moving. Does have a history of Type II diabetes for which she is supposed to take an oral medication. Unfortunately, she does not take her medication as prescribed. She came to clinic yesterday for a routine check-up. The midwife noted that the baby seemed to be large for a nearly-term baby and the decision was made to induce labor. Baby was delivered vaginally without difficulty. On exam, she is large-for-gestational age but has a normal face (not dysmorphic) and resembles other family members.
