Situation – Astra Parker has been noticing an increase in shortness of breath when doing simple tasks around the house and has noticed some “heart skipping”. She comes to the ED because she now feels it is happening more frequently and she is now noticing she has a gnawing pain in the middle of her upper back. Background- Astra is a 69-year-old female with a history of hypertension. She is a non-smoker and has recently changed to a vegan diet. She is careful with her diet and started walking every day to increase the amount of exercise she gets every day. She has been on a beta blocker medication which has controlled her blood pressure with her last BP 130/75 at her last doctor’s office visit. Astra states that she takes – Atenolol 20 mg orally daily, MVI 1 tab orally daily, and aspirin 81 mg orally daily. Assessment – The patient is awake, alert, and oriented X 3. PERRLA, moves all extremities to command, and exhibits equal strength bilaterally. Exhibits shortness of breath while lying in bed; lung auscultation reveals bilateral crackles in the bases that do not clear with coughing. Bowel sounds normal; abdomen is nontender to palpation.
