Per examination: Grace is well-hydrated, well-nourished but agitated appearing female – appropriate for age, in significant distress with speaking a single word at a time and with an audible expiratory wheeze. Her cardiac exam is unremarkable. Auscultation of her lungs reveal moderately decreased air exchange in all fields with inspiratory and expiratory wheezing with air exchange. No crackles or rhonchi appreciated. No grunting or nasal flaring noted. Tracheal retractions are appreciated. Her POX is 93% on room air. HEENT shows boggy purplish, congested nasal turbinates bilaterally. clear PND with mild cobblestoning of posterior pharynx. Bilateral TMs clear. Shotty posterior cervical lymphadenopathy noted. Remaining HEENT exam WNL.
