A 30-year-old man was admitted to the intensive care unit unconscious. Patients wife said that he has a severe form of type 1 diabetes, he is treated with insulin therapy in a daily dose of 48 units. Diabetes mellitus was well controlled. Patient played badminton with friends and after 30 minutes he lost consciousness. Physical examination: consciousness is absent. The skin is moist, muscle tone is increased, tonic cramps and masticatory trismus are present. Pupils are dilatated. Meningeal signs are negative. Tongue is moist. Breathing shallow, no wheezing. Respiratory rate 24 per minute. Heart: rhythm is regular, muffled sounds, BP 130/70 mmHg, heart rate 90 per minute. Laboratory tests: plasma glucose – 2,1 mmol/l, urine acetone is negative. 1.
