Client is on 2 L oxygen by nasal cannula with oxygen saturation of 92%. Auscultation of lungs reveals scattered wheezes and rhonchi bilaterally. Client is short of breath, and is sitting upright. Client is coughing up thick green-yellow sputum. Client’s skin is warm and pale, with brisk capillary refill. Client’s abdomen is soft and nontender, and bowel sounds are heard in all quadrants. The skin around the client’s abdominal peritoneal dialysis catheter site is intact, and not tender or red. No drainage around catheter is noted. Client is concerned that peritoneal dialysis needs to start soon to get three exchanges before bedtime. Client performs own dialysis at home but will be unable to do it now due to shortness of breath. Client states only performing peritoneal dialysis for about 4 weeks and has many questions. In particular, the client’s concerns focus on cleaning and handling the catheters, skin care around the abdominal catheter insertion site, and dietary restrictions. Vital signs: heart rate, 88 beats/min; respiratory rate, 24 breaths/min; blood pressure, 134/80 mm Hg; temperature, 99.4°F (37.4°C).
