John is a 46-year-old truck driver who typically drives a few days a week away from home and comes home once a week to his wife and kids. His wife is a stay-at-home mother, and they have four children, a daughter who is twelve, twin boys who are eight, and a nine-month-old son. His older daughter enjoys gymnastics and dancing, while his boys love swimming and karate and practice at the YMCA in their community. As a long-haul truck driver, John spends hours behind the wheel and almost always orders high-calorie fast foods via drive-through for his convenience. John has been a smoker for the last 16 years and has struggled to keep his weight off lately. He has no time for exercise when he comes home and lives a sedentary lifestyle on his days off. 
John can only come home to his wife and kids twice a week since he is the primary breadwinner. His daughter seems to detach herself emotionally from her dad since he is away from home most days. His daughter’s behavior has been bothering John for the last few weeks. Since it was the week of Christmas, John had a week off from work. One night in bed, his wife noticed John had some swelling of the belly area and was short of breath lying down. When his wife questioned John, he said he realized he felt full most of the time and was huffing to breathe even short distances. John has a history of hypertension and hyperlipidemia. He was diagnosed five years ago but is non-compliant with taking his meds due to the cost and his lifestyle. As his symptoms worsened daily, he started experiencing edema on his lower extremities and had been experiencing shortness of breath with little activity. Due to these symptoms, John had to take off work, significantly impacting his financial situation in the house. John and his wife have been very stressed recently, affecting care for his kids. Finally, after insisting on visiting the doctor, John decides to see one. His labs came back to confirm the diagnosis of congestive heart failure.
