Instructions: Think of the intervention’s nurses do in their work every day!  How do these interventions fit with the ANA’s 8 Competencies for Care Coordination?  Identify some of the many “care coordination interventions” that we are doing in our nursing practice every day!

Read through the 8 competencies in the ANA’s “Standard of Practice 5A”, which are listed on this template below and on page 46 in the textbook. (For e-book users, see the chapter on Action Issue 2.)
Respond to 2 of the 8 competencies on this template, fully describing and giving an example of each competency.  
Describe what actions a nurse might perform in their work setting that demonstrates that competency. Explain at least 2 nurse interventions for each competency selected.  (See grading rubric below.)
Your posts can describe something specific a nurse did/does that demonstrates that competency, or something nurses do in general that demonstrates that competency. The goal is to identify the things that nurses do in their work that would be considered “care coordination!” 
This assignment will count toward your Care Coordination Activities grade.

Select 2 of the 8 competencies listed below and describe each competency fully and give an example. For each care coordination competency, you selected and described, discuss care coordination interventions associated with each competency. List at least 2 interventions that may be used for the given example for each competency.
