Albert Brooks is a 35-year-old patient.
He has a history of depression and alcohol abuse.
4 days ago, Albert fell down his stairs at home while intoxicated and sustained a large,
deep cut to his left hip.
The wound was cleansed and closed with staples. He was
discharged from the hospital after 24 hours.
2 days ago, on Oct 25, he was readmitted
through the ER, when he fell again and the wound on his left hip dehisced.
He waited several hours before calling the ambulance.
It is the beginning of your 12-hour day shift on Oct 27.
You have received shift report
from the previous LPN.
Albert is on CIWA protocol and has been experiencing significant nausea. He has not
vomited but complains of retching.
Albert has no complaints of auditory or visual
disturbances, however there is a slight tremor noted bilaterally when arms are raised.
Albert is diaphoretic with moisture noted on his gown and forehead.
The sheets are dry.
Albert states “I just want to get out of here”.
Albert smokes 2 packs of cigarettes a day.
You note there is a nicotine patch on his left upper outer arm. It is dated Oct 26 at 1600.
Recent lab results indicate he has decreased kidney function.

You have been provided with the CIWA protocol, CIWA assessment form, shift report,
physician’s orders and intake/output sheet.
You will complete your assessments and any interventions necessary.
You may need to collaborate with the RN or call the physician. Your instructor will act as
either the RN or the physician when this is necessary.
You will need to decide if there are any other documentation forms required.
