with his dog. He had bilateral cataract removal several years ago. He usesÂ
MiraLAX® for periodic constipation but takes no regular prescription medications. He doesn’t
smoke or use illegal substances. He typically has a glass of wine or a mixed drink in the evening.
He enjoys sexual activity and denies problems with pain, hearing, vision, appetite, bowel,
bladder, or sleep. He still works part-time as a financial consultant and denies issues with
memory and cognition. He states that he has never experienced anxiety or depression, except for
situational depression that occurred with the loss of his first wife.
