All should be involved for safety and that it is a component of quality management. Most organizations are careful not to mix the patient charting with the quality management system. In our institution, we use a computerized reporting system that is separate from the computerized patient record. The individual will report the incident, putting in the necessary data. The patient safety coordinator and the manager receive copies and follow up on the issues.Â
However, we have noted that individuals are sometimes hesitate to initiate incident reports. Sometimes individuals may think that they will be in trouble or blamed for whatever happens to the patient. Why is it important to have a blame free environment, and how does one go about creating one?
