Sarah is a 15 year old freshman in high school admitted to your unit in sickle cell crisis.  Sarah explained that she started to develop pain in her left lower extremity early this week and got to the point where she couldn’t handle it anymore.  Prior to the pain, Sarah explained how her softball team has been having daily practices leading up to the sectional tournament starting next week.  She explained when she is not playing softball she is studying for her finals.  She is worried about finals because she wants to make good grades so colleges will be interested in her.  She plans to be a pharmacist and says how competitive the program is at Butler and needs to make good grades.

Neuro: Alert and oriented x4, speech clear and coherent, follows commands

Cardiac:  Regular rhythm, HR 78, BP 108/72 automatic, non-invasive RUE, distal pulses palpable at 2+, D5 ½ NS running at 2x daily maintenance.

Pulmonary:  Breath sounds clear bilaterally, RR 18, Pulse Ox 93% on room air

GI:  Bowel sounds x4; soft, flat, non-distended, non-tender, last BM stated as yesterday

GU:  No urine assessed at this time; last void in ER

Skin:  warm, dry, normal for ethnicity with pink undertone

Pain:  9, LLE, described as throbbing / aching, last dose of Morphine given 2 hours prior in ER.
