DP is a 66-year-old divorced AAF who comes to your practice for her Welcome to Medicare Visit. She is divorced and recently returned to work part-time time at the local library to supplement her pension from the Archdiocese of Chicago where she worked as a lunchroom attendant. She did not pay into Social Security and recently learned that she is entitled to a pension and Medicare as she was married to her former husband for over thirty years. She has a male companion and admits to being sexually active. Family history includes father deceased from a stroke at age 50 years, mother living with hypertension and type2 diabetes, a sister(deceased) with breast cancer and a brother with pancreatic cancerand CAD and end stage renal disease secondary to Type 2 diabetes DP has not seen a health-care provider for several years as she had no health care insurance. Now she has Medicare, so she is coming in for care. She is a former smoker who quit 5 years ago after smoking 1 pack per day since age 20. She has four grown children, all of whom live nearby, and she has 8 grandchildren.
