A 39-year-old woman develops calf tenderness and shortness of breath after flying from Rome to New York City. Her health has been good, and an oral contraceptive is the only medication she is currently taking. Physical examination reveals a woman who appears moderately ill and is short of breath at rest. Vital signs are: temperature 97.4 °C, blood pressure 125/70 mm Hg, pulse 88 beats/minute and regular, respiratory rate 22 breaths/minute, and pulse oximetry is 90% on room air. There is tenderness in the mid-calf region on the left. There is no edema or induration. You obtain duplex Doppler ultrasonography of her left lower leg. An extensive thrombus is noted from below the knee to the iliofemoral vein. As part of her workup, you also obtain electrocardiography (ECG).
