This 84-year-old presented to the ED after having a past medical history of CHF, atrial fibrillation, and hypertension. She is also legally blind as a result of macular degeneration. She also has a history of CAD, and medications include nitroglycerin, metoprolol, Norvasc, Lasix, Tagamet, and Meclizine. Vital signs are normal. HEENT: Atraumatic, normocephalic. Eyes show S/P iridectomy. TMs, pharynx and nose are normal. Neck: Supple, no lymphadenopathy Heart: S1 and S2, irregular Chest: Bibasilar rales Abdomen: Soft, nontender Extremities: 2+ pedal edema Labs normal. CT of head no bleed. U/A showed 3+ bacteria.
