Client, who traveled to Africa 1 month ago, reports having a cough productive for moderate amount of thick yellow blood-tinged sputum, fatigue, low-grade fever, and night sweats for 5 days. Client has been taking acetaminophen and an over-the-counter (OTC) antitussive with relief until this morning, when client experienced nonradiating chest pain with coughing and primary care provider admitted client to hospital. Pain does not worsen with deep inspiration or leaning forward. Scattered crackles heard in lungs bilaterally. Symmetrical chest wall movement with breathing, normal anterior-posterior chest diameter. S1 and S2 heart sounds heard on cardiac auscultation, no murmurs or rubs noted. Vital signs: temperature, 100°F (37.8°C); heart rate, 76 beats/min; respiratory rate, 22 breaths/min; blood pressure, 138/74 mm Hg. Oxygen saturation, 95% on room air.
