The first thing I noticed was the patient’s respiratory rate was 24 breaths per minute which is much higher than the normal range of 12 – 20 breaths per minute. Since the patient is experiencing tachypnea, many different orders can be anticipated. According to the National Institutes of Health, “Providers can evaluate based on oximetry, arterial blood gases, chest x-ray, chest CT, pulmonary function tests, glucose, electrolytes, hemoglobin, EKG, VQ scan, brain MRI and/or a toxicology screen” (National Library of Medicine, 2021). Therefore, we could expect any combination of these to be listed in the medical orders. The second thing I noticed on the patient’s labs was that she was experiencing hypokalemia with a potassium level at 3.0 mmol/L when the normal range falls between 3.5 – 5.0 mmol/L. With a patient who is hypokalemic and has been vomiting for three days an anticipated medical order would include intravenous potassium replacement therapy. According to the National Library of Medicine, “potassium should be given with close follow up, continuous ECG monitoring, and serum potassium level measurements” (National Library of Medicine, 2018). The next order I would anticipate for this patient is the use of an incentive spirometer specifically due to her medical history and current respiratory issues. According to the Cleveland Clinic, an incentive spirometer can “help strengthen the lungs and improve lung ventilation” (Cleveland Clinic, 2022). Finally, with the patient vomiting I would expect intravenous fluids to be listed under the medical orders to help correct the fluid electrolyte imbalance.
