Carbamazepine, also known as Tegretol, is a medication used primarily in the treatment of epilepsy and neuropathic pain. It has a Black Box warning, which is the strictest warning put in the labeling of prescription drugs or drug products by the Food and Drug Administration (FDA) when there is reasonable evidence of an association of a serious hazard with the drug. In the case of Carbamazepine, the Black Box warning is due to the potential development of Stevens-Johnson Syndrome (SJS). SJS is a rare but serious disorder that begins with flu-like symptoms followed by a painful red or purplish rash that spreads and blisters, eventually causing the top layer of the skin to die and shed. It’s a medical emergency that usually requires hospitalization. The risk of developing SJS is higher in patients with a particular genetic variant common in some Asian populations. Therefore, genetic testing is recommended before starting treatment with Carbamazepine in these populations.
