Prepare the above memorandum Interoffice Memorandum/To Department Manager – Gary/From Karen Larsen MD/Current date/Subject Outside Laboratory Usage/ After careful study, I have decided that Quest Diagnostic Laboratory will be our outside resource Laboratory for the next three months. They have contracted to provide us with fast, reliable service. They are certified by Medicare to provide all necessary lab test results. Our contact person at Quest will be Gina Davis. She will bill us directly for any outside services we use with Quest. Also, she will send us monthly reports on our usage of their facility. Gary, I want you to keep an accurate report of turnaround results and other possible problems encountered with the lab tests we send to Quest. During the week of _______________, we will have a meeting to discuss our usage of Quest. You and Gina will meet with me to discuss the continued usage of Quest Diagnostics.
