History of Chief Complaint Location: The pain is located at the base of the neck near the CT junction. Cannot tell if right or left side is worse Onset: 2 weeks prior, was visiting his kids in NY. In the pool he was trying to launch one of his kids from his shoulder and she slipped and fell on his bent neck. He describes the action as: he was squatting under water and his kid stood on his shoulders. He then rapidly stood up and she typically jumps to the deeper water. Chronology: Felt immediate pain in the back of his neck. Got out of the pool and iced it. It seemed to feel a little better the rest of the day but the next morning the pain was extreme. Went to an MD in NY, had x-rays (negative for fracture), was given a muscle relaxer and told to take max dose ibuprofen for 4 days. Got back to Portland and his neck pain has subsided a lot but it is still painful. Quality: The pain is described as dull and achy most of the day and occasionally gets sharp with certain movements. Severity: OPS 4/10 constant 7/10 when experiencing
