A 63 year old man has been admitted to the unit with a diagnosis of chronic obstructive pulmonary disease (COPD). He states that he has a great deal of difficulty breathing while walking, even a short distance. “It’s hard for me to get anything done”. He also states that his heart feels like it is racing at the same time. He is constantly coughing up large amounts of thick, tan mucus in large amounts but gets very tired doing so. He states that he is tired all the time and while talking to you he is continually wringing his hands, tapping his foot and looking out the windows. Pt states “Sometimes I feel like I might take my last breath. How am I supposed to live like this?”. Patient has clubbing, slight cyanosis of fingertips, crackles at bilateral bases of lungs and rhonchi throughout. 1. Identify at least 3 actual problems this patient has. 2. Formulate a nursing diagnosis for all 3 of the patient’s problems. 3. Formulate one goal for each NANDA. 4. List at least 5 interventions for each NANDA.
