Journal? Subjective Data Patient information: Fred Chief Complaint: “I have got this rush that I need to medication for. The rush started about two days ago as an itchy feeling but yeaterday I noticed some blisters. However, today the rush seems to be spreading.” History of present illness (HPI): Fred presents with a rash on his arm that started two days ago, initially red and itchy, progressing to blisters. It has spread and worsens after showering and at night, interfering with his sleep. Past Medical History: Deferred Family History: Deferred Immunizations: Deferred Allergies: NKDA Medications: Saw Palmetto 1 tab qd, Keflex 500 mg 1 tab BID, and TUMS 1 tab prn Habits: Fred has been fixing fences outside for the last couple of weeks, wearing short sleeves due to the hot weather. Review of Systems (ROS) General/Constitutional: No fever, chills, or chest pain reported. Skin: Rash present on the arm, itchy, with blisters, spreading, and worsening after showering and at night.
