Carl is a 57-year-old African American male who presents to the emergency room via ambulance with a new onset seizure. The paramedic states that responded to a call at a local restaurant where it was reported that Carl was observed to have “passed out and started shaking violently.” His wife states that he had ordered his food and then told her, “I don’t feel quite right,” before he seemed to lose consciousness and fell out of his chair, with his arms and legs extended before shaking violently. He did not breath for a period of approximately 15 seconds. His wife states that he has been losing some weight lately—”about 5-10 pounds in the past month or so,” despite having a good appetite. He has complained to her about feeling tired over the past couple of months. Carl states that he has had a headache almost every morning when wakes up for several weeks and has been taking ibuprofen with some relief. His wife states that she has tried multiple times to get Carl to see someone, but he is “hard-headed” and won’t go.
