LOCATION: Inpatient, Hospital PATIENT: Grace Hargrove PHYSICIAN: George Orbitz, MD PRIMARY DIAGNOSIS: Acute peritonitis due to peritoneal dialysis. SECONDARY DIAGNOSIS: End-stage renal disease, hypertensive, on chronic peritoneal dialysis for chronic renal failure. HISTORY OF PRESENT ILLNESS: The patient is a 79-year-old white female who is known to have end-stage renal disease on CAPD (continuous ambulatory peritoneal dialysis). HOSPITAL COURSE: She was admitted with abdominal pain and cloudy dialysate. She was found to have alpha streptococcal peritonitis. She was given vancomycin, gentamicin, and Kefzol. The Group A strep was sensitive to cephalosporins. She did well during her hospitalization and felt better. DISCHARGE PLAN: The patient will be discharged home today. Mario, from dialysis, will teach her how to add antibiotics to her peritoneal dialysis bags. The patient agrees with the plan. She was advised to call us back if she has any more problems.
