CC: 20-year-old Asian female with itchy rash on neck for 4 days HPI: Rash on neck started four days ago and is only on the front neck area. It’s itchy and red all the time. The more she scratches the worse it feels. She has not tried anything on the skin to make it better other than moisturizer. Nothing but scratching makes it worse. No one in the family or at school has a similar rash. No recent illnesses, no pets or travel. She did just get a new necklace from her boyfriend for her birthday two weeks ago. And she did recently change to Caress soap about a month ago. PMH: Mostly healthy, childhood asthma now asymptomatic for five years has seasonal allergies especially in Spring. No history of hospitalizations or surgeries. Meds: Lutera oral contraceptive pill x 1 year and amultivitamin daily. FH: She currently lives away at college for one more term, otherwise lives with parents who are alive and healthy. She has two older brothers aged 22 and 25 who are healthy.
