According to the FDA, approximately 1.3 million Americans a year are killed or injured due to medication errors. Many of these preventable mistakes are caused by incorrect medication dispensing and administration, often due to mislabeled packaging. The Agency for Healthcare Research and Quality notes that these errors occur at all stages of the medication-use process, including ordering, transcribing, dispensing, and administration. One widely discussed method of reducing errors during the “administration” phase is bar-coded medication administration (BCMA). BCMA connects implementation of an information technology solution called “Electronic Medication Administration Record” (eMAR) with item-specific identification (bar-coding). BCMA helps users to be in compliance with the “Five Rights” of medication administration: right patient, right dose, right route, right time, and right medication.”). BCMA was designed to augment patient safety in the administration of medication by providing checks and safeguards, but was not intended to be a replacement for the clinical judgment of the medication administrator.
