CC: “I am having this cough that won’t go away” HPI: Marge is 58 years old and presents with fatigue and difficulty catching her breath. The patient states that she has coughed frequently for about 4 months, which she thought might be allergies, but the fatigue and shortness of breath have come on over the past 2 months. She states that her current cough is sometimes productive of white sputum, but not always. She has tried over the counter Robitussin with no relief from her symptoms. She states that she has not had trouble sleeping and that the cough is occasional, but persistent. When asked about any previous symptoms in the past, she responds, “Not exactly,” and then says she was treated twice for bronchitis in the last year. Both bouts included a severe cough accompanied by shortness of breath, but they also included yellow sputum and fever each time. She was prescribed antibiotics both times, and by the time the prescription was completed, the shortness of breath resolved, and she was doing fine until recently. She is not a current smoker. She was able to quit successfully on her 58th birthday (6 months ago), after a 40-year history of
