Newborn Small Group Discussion-Sam, a 4 day old newborn presents to your office for a well baby exam. He was born on 10/20/20 at 1604. His appointment with you is on 10/24/20 at 0830. PMH: Sam was delivered at 37.0 WGA via SVD to a G2 now P2 23 yo Hispanic female. Pregnancy was complicated by gestational diabetes. He had a normal antenatal and newborn course otherwise, passed hearing and CHD testing in nursery, received HepB #1, Vitamin K, and erythromycin ointment in nursery. Birthweight was 2862 grams, length 19 inches. Had a serum bili drawn in the hospital which was 7.1 at 27 hours of life. He was discharged at 40 hours old. Discharge weight was 2740 grams. No ABO incompatibility. Hx: Sam lives with his parents. His birth father is involved but works long hours. MOC states that her “milk is in.” However, she reports concern today for cluster feeding “all the time.” Reports exhaustion today. He is nursing every hour but usually falls asleep at the breast and MOC has to wake him. He has 3 wet diapers and 2-3 stools daily, wakes to feed hourly overnight. MOC appears exhausted today.
