The assessment findings that indicate dehydration may be present is the client’s skin is dry and flaky and tents on slight pinch, oral mucosa is dry, cap refill more than 3 seconds, the Fowley bag after five hours only having 45ml’s of urine collected and confusion (Mayo foundation for medical education and research, n.d.). The laboratory findings indicate dehydration is the Na level in the patient of 165 mmol. This level of Na in the body indicates a condition known as hypernatremia (Kim, 2006). Hypernatremia means that the body doesn’t have enough water compared to its sodium stores. This can happen if the body loses water or gains sodium too quickly and causes symptoms such as muscle weakness, being unable to rest, nausea, mood changes, exhaustion, and confusion. The assessment findings that indicate poor nutrition is the client appearing to be thin and reported he did not feel like eating after his wife died and that his right arm “not working very well” makes it hard for him to open jars and cans.
